Covid-19 screening survey
We’re screening all patients for COVID-19 symptoms before attending Spire hospitals. Please read the instructions carefully before completing.
Please follow the instructions in the survey and answer ALL of the questions. If a question is not applicable to you, please answer N/A. Based on your answers to the survey, we may need further information from you or ask you to arrange a COVID-19 test and reschedule your appointment.
Please note: If your appointment is taking place as a virtual consultation there is no need for you to complete this survey.
Come to your appointment alone: In line with national guidance visitors entering the hospital remains restricted. Prior approval for visitors or relatives to attend is required from the Director of Clinical Services.
Face masks: It is a mandatory requirement for patients to wear a mask within our hospitals unless medically exempt.
To allow social distancing in our waiting areas please arrive five minutes before your appointment time.
