I spent eight years as a specialist registrar in recognised acute posts on SE Thames SpR rotation including two years as lecturer in surgery which had an on-call commitment. The first three years of the rotation involved a very general training, gaining experience in all the major sub-specialties.
Years four and five were spent conducting clinical research in the oesophageal laboratory at Guy’s using the techniques of manometry, ambulatory pH and bilirubin monitoring leading to the MS degree. The final three years were focused on gastro-intestinal surgery, achieving level 3 training a St Thomas' and Brighton in upper GI surgery. Since 2002 I have been a consultant general and upper gastrointestinal surgeon in Brighton.
Upper GI surgery is my main sub-specialty interest and was my declared sub-specialty at the InterCollegiate Exam. I have performed over 200 oesophagectomies (radical transhiatal, left thoracoaabdominal, Ivor Lewis and three stage approaches) and total gastrectomies for cancer and have assisted in many more. Many of these cases underwent pre-operative staging laparoscopy which I either performed or assisted at.
At Guy's, St Thomas' and Brighton I have participated in regular multidisciplinary meetings where the management of these upper GI cancers has been discussed and decided. I have significant experience with anti-reflux surgery surgery (Nissen, Toupet, paraoesophageal herniae) and revisional surgery around the hiatus.
I have an established practice of laparoscopic anti-reflux surgery. I have surgically dealt with the majority of upper GI emergencies (Boerhaaves, bleeding/ perforated ulcers, gall bladder disease) and in Brighton I participate in the on-call rota for therapeutic endoscopy performing injection therapy/ banding for bleeding varices and ulcers.
I am confident in managing gallstones disease and benign conditions of the pancreas. I have no personal experience of hepatic resection. I am experienced in all aspects of performing and interpreting investigations of oesophageal function obtained during my period of research undertaken in the oesophageal laboratory at Guy's.
I am currently surgical clinical lead in he Digestive Diseases Department. I have been the general surgical representative on the Medical Advisory Committee of the Brighton Nuffield Hospital for several years now.
