If you would like to obtain a copy of your medical records please write to the Medical Records Department at Spire Little Aston Hospital state your full name, date of birth and address. If your address was different at the time of your treatment please advise us of this. Please ensure your letter is signed using your usual signature as we will verify this against signatures in your patient record to confirm your identity.
If you only require a specific part of your medical record please advise us of this.
On receipt of your request, we will write to you confirming the costs payable for photocopying and postage (up to the maximum £50.00 charge under the Data Protection Act 1998). Once we receive payment we will ask the clinicians involved in your care to give their consent for release of your records to you. This may take up to 40 days. Once the copy of the medical record is ready to be sent to you it will be posted by recorded delivery.
Please note we will only be able to send a copy of your medical records held at Spire Little Aston Hospital. If you have received care elsewhere and also require these records you will need to write separately to these hospitals/clinics.
Requesting copies of the records of another adult patient
If you are requesting a copy of the medical record on behalf of another adult patient please follow the process above and enclose a letter of consent authorising the release of records from the patient.
Requesting copies of the records of a deceased patient
If you are requesting copy medical records of a deceased patient or have any questions relating to requesting medical records please contact the hospital’s Matron via switchboard on 0121 353 2444.
