What you get with Spire Diabetes Care

For £9.95 a month, you will receive consistent, ongoing support that starts with an in-depth, face-to-face consultation with your diabetes support nurse at your Spire Diabetes clinic.

During your consultation, you will have a blood profile and urine test and your nurse will explain in greater detail what to expect from the Spire Diabetes Care service and discuss any concerns or challenges you're facing with your pre-diabetes or diabetes.

You'll also receive: 

  • A comprehensive review of your recent medical history, including a review of your NHS Summary Care Record
  • Easy-to-use equipment to allow us to monitor your weight and blood pressure
  • A demonstration of how to use your digital weighing scales and blood pressure monitor
  • Help setting up the diabetes App on your smartphone (minimum requirement iOS 12+ or Android 5.1+)

How we can help you

Regular, one-to-one care, including monitoring and feedback, from your diabetes support nurse and Spire GP, can support you to make positive, proactive lifestyle changes that help keep your type 2 diabetes under control and help you better cope with the emotional challenges of living with diabetes.

Monthly support and monitoring

After the initial consultation with your diabetes support nurse, you'll be asked to record your weight, blood pressure and mental wellbeing every month via the diabetes App. You will then receive:

  • Monthly personalised emails from your nurse: to assess your body weight and blood pressure measurements, mental wellbeing, and progress towards your goals
  • Bi-monthly face-to-face or virtual meetings with your nurse: to discuss your progress and provide further tailored support, including foot checks at your face-to-face meetings
  • Blood profiles and urine tests twice a year: tests will be performed at your Spire Diabetes clinic and will help monitor your blood sugars, kidney function and cholesterol
  • Two virtual appointments each year with a Spire GP: to discuss your progress and blood and urine test results

Your diabetes support nurse will be supported by a Spire GP. Any blood test results, significant updates on your health status or medication changes recommended by the Spire GP will be communicated to your NHS GP practice on your behalf. You will receive support for a minimum of 12 months which can be extended for as long as you need us.

How do I know if Spire Diabetes Care is appropriate for me?

Complete our questionnaire and answer the questions as accurately as possible. You won’t need to provide any personal details and will receive an answer as to whether this is appropriate for you immediately upon completing the questionnaire.

If you choose not to subscribe to Spire Diabetes Care, all of your questionnaire responses will be deleted.

How long do I need to commit to Spire Diabetes Care?

The Spire Diabetes Care service has an initial minimum term of 12 months. This will allow you to get the most out of Spire Diabetes Care by giving you and your nurse the time to identify positive changes you can make to your lifestyle, diet and general health, as well as track the impact of these changes and your progress.

How can I cancel my subscription?

Can you offer support for individuals with type 1 diabetes?

Spire Diabetes Care is specifically designed for individuals with type 2 diabetes and, therefore, does not offer support for individuals with type 1 diabetes.

Do you supply any equipment I might need? Will there be any help or instructions?

Yes, during your first appointment with your nurse, which will be conducted in person at your Spire Diabetes clinic, you'll receive a blood pressure monitor and digital weighing scales. Your nurse will demonstrate how to use them and will provide you with instructions to take home. Your nurse will also help you set up the diabetes App on your smartphone and show you how to enter your weight and blood pressure measurements into the App.

What happens if I change my NHS GP?

If you change your NHS GP but remain at the same NHS practice, you don’t need to tell us or take any further action regarding your Spire Diabetes Care service. All updates we provide to your NHS practice will continue to be added to your medical records.

I can’t travel to Central London. Will the service be available from other locations?

For the pilot of Spire Diabetes Care, all face-to-face appointments will be conducted at a central London clinic, with the remaining appointments conducted virtually via Zoom. We hope to expand the service across the UK in due course.

If I subscribe to Spire Diabetes Care but then find I can no longer afford it, what are my options?

Subscription to Spire Diabetes Care requires a minimum 12-month commitment, after which you can cancel your subscription with a month’s notice. If your financial circumstances change during the initial 12 months and you're struggling with your monthly subscription payments, please email us at Spirediabetescare@spirehealthcare.com.
