My current post is consultant rheumatologist at Warrington Hospital Trust. I run in-patient care and out-patient services with the support of an SPR (senior registrar), three full time clinical nurse specialists, two physiotherapists and occupational therapist. I have established closer links with the general practitioners and introduced share care programme. Management protocols for rheumatic diseases were produced with participation of GP's. I have worked in the multidisciplinary team and appreciate the strengths in providing care for patients with chronic disease. The liaison with physiotherapists, occupational therapists, clinical nurse specialists, meteorologists and social workers are essential in providing a service for patients.

I have spent more than seven years in rheumatology training as an SHO, registrar, ARC research fellow, senior registrar locum and lecturer. During this time I had the experience of managing a variety of chronic and acute rheumatic conditions arising from complicated connective tissue diseases, sepsis, vasculitides and all aspects of musculoskeletal medicine.

Managerial positions held: head of the rheumatology department, examiner and chair for the PACES MRCP clinical examination for the Royal College of Physicians, chairman of R&D in Warrington NHS Trust, member of rheumatology STEC, member of pathology users group at Warrington Hospital and rheumatology specialist lead for the trust.

Research and publications

I have written my MD thesis on immunological aspects of peripheral neuropathy in rheumatoid arthritis. My research involved detection of antibodies against peripheral nerve antigens, such as gangliosides and neurofilaments, in rheumatoid arthritis patients who develop peripheral neuropathy.

We detected anti-ganglioside antibodies in 43% of patients with RA complicated by peripheral neuropathy using an ELISA technique that we have developed. These serological markers are already viewed as useful indicators for nerve damage in RA. Autoantibodies to peripheral nerve antigens are detectable in synovial fluid and may contribute to the articular damage in RA. Immunofluorescence and western blotting studies of neuroblastoma cell lines have demonstrated a specific staining pattern and common antigen reactivity of sera from RA patients  with peripheral neuropathy. We have investigated vascular endothelial injury in this subgroup of RA by developing methods for detection of anti-endothelial cell antibodies and by analysing expression of cell adhesion molecules.
